How Our Stabilization Model Works
Rachel Ward, PMHNP-BC leads our complex patient intake. Rather than a traditional long-term case load, Rachel specializes in shorter-term (typically 6 months to a year) stabilization - working intensively with new patients to establish an accurate diagnosis, builds a treatment plan, and create real momentum. When appropriate, she transitions patients to one of our other providers for ongoing care.
For your patient: Expert psychiatric care quickly, with supported handoff built in from the start.
For you: We collaborate closely with outside therapists throughout. You won't be left in the dark.
Please Confirm Before Referring

The Patient is stable enough to not require provider access during evening or weekends

The patient does not require wrap-around services or intensive community support

The patient is able to engage in scheduled outpatient appointments.
If your patient currently needs a higher level of care, we recommend PHP, IOP, or Crisis Stabilization first. We welcome your referral once they have stepped down appropriately.
If a patient destabilizes after intake, our team responds by increasing appointment frequency, coordinating with the referring therapist, and referring to a higher level of care when clinically indicated. In a crisis, we direct patients to the nearest emergency room. With patient consent, we will notify you of any significant changes in care status.
